OSF HealthCare is pleased to add a new primary care provider to its staff to better serve Sheffield and the surrounding areas.
The addition of Sydney Lebahn-Franks, APRN, on July 6 will ensure increased access for patients in need of primary care at OSF Medical Group – Primary Care, located at 240 N. Mason St. in Sheffield.
Lebahn-Franks offers a number of primary care services including preventive care, mental health care and chronic disease management.
She received a Bachelor of Science from St. Ambrose University her Master of Science in Nursing from Northern Illinois University.
