OSF HealthCare is honored to be named one of “50 ACOs to know” by Becker’s Hospital Review. This distinction recognizes the results we’ve delivered since becoming an Accountable Care Organization (ACO) in 2012 and the goals we continue making progress toward.
How the program works
An ACO is a group of doctors, hospitals and other health care providers who come together to provide coordinated care to their patients. When care is coordinated, all of these providers can see the services that a patient has received. This ensures that patients, especially the chronically ill, get the right care at the right time while preventing medical errors and avoiding unnecessary duplication of services.
In this value-based care model, OSF is paid based on the quality of outcomes we help achieve for patients and the Medicare savings we deliver for U.S. taxpayers.
